From this page, you can link to the following financial forms: two letters regarding tax-deductible contributions, and a check request form. 

To find the team's registration forms, click here to go to "Join the Team" and there you will find all the registration forms, which you can print and fill in. 

IRS Contribution Letter for Monetary Donations: Click here for the letter you need to fill in to claim a monetary contribution to the team.  All families paid a tax-deductible contribution of $175 to the team as part of the initial registration fees. 

IRS Contribution Letter for In-Kind Donations:  Click here to find the letter you can use to document in-kind contributions to the team. 

Check Request Form:  Click here for the check request form.  If you have a pre-approved expense for which you wish to be reimbursed, fill in this form and give it, with your receipts, to the team treasurer, Caryn Pearson.
